Story & Diet uses online history and story review. Assessment & Diet adds one clinic or eligible van assessment per month. Weekly Nutrition Care includes weekly reassessment in an agreed format.

Know who may see the plan

The patient’s permission or appropriate representative authority is needed for sharing personal health information. Booking or paying does not automatically give access.

Extra laboratory or genetic tests need a reason, consent and separate pricing. Urgent concerns should go to your treating team without waiting for a routine nutrition review.
