The information is used to review and coordinate the request you choose to make. It is stored in the website’s request records and is not displayed publicly. Submitting a form does not subscribe you to marketing or create permission to publish a story.
What not to send here
Do not enter medical reports, detailed diagnoses, genetic data, identity documents, passwords, payment-card information or another person’s confidential records. These forms have no clinical-report upload or payment facility. Ask the professional for the appropriate care-record channel if needed.
Caregiver and representative permissions
Obtain the patient’s permission or appropriate authority before arranging care for them. Use the coordinating person’s contact details. Permission to book or pay does not automatically allow access to history, results or the diet plan. The patient’s wishes, capacity and any representative authority must be addressed in the actual care process.
Optional tests, photographs and genetic information
A general website checkbox is not consent for a blood draw, genetic test, clinical photograph or publication. If an optional service is offered, ask who collects and processes the sample or information, why it is needed, who can receive it, storage and deletion arrangements, and the choices if you decline.
